Middle School Field Hockey: Important Information

July 15th, 2014

The sign up’s for middle school field hockey will be on Friday, September 5th from 2:30-2:45 in High School Room 27. At the meeting the girls will receive all paperwork that must be filled out in order to participate. On the first day of school there will be signs on the middle school bulletin boards and in the middle school hallways posting information about the meeting. Practices will start September 11th. In order to participate in the first practice all paperwork must be completed and your child must have a physical. At the meeting all the necessary paperwork will be handed out and each form will be explained in detail. It is important to schedule your physical appointment this summer! You can print the physical exam form out on the school website under athletics. If you do have your child get her physical this summer please make sure the doctor fills out the information on the schools physical form or it will not be accepted by the school. Middle School Girls Soccer

Attention all middle school girls interested in playing soccer for the school this fall. There will be a mandatory meeting on Friday, September 5, 2014, at 2:30 p.m. in high school room 8 (across from the media center). All girls who wish to play must attend the meeting.

Because our season begins so early this year, it is very important that you have ALL paperwork in to the school NO LATER than the Friday, September 5, 2014. This includes the following: Physical Part A and Part B, IMPACT testing form, Pay to Participate, and the Uniform Code of Conduct. You may turn in your physical forms in the front office anytime during the summer if they are complete. DO NOT hand in the IMPACT testing or Code of Conduct to the front office. You may turn in all other forms to Ms. Serzan in high school room 8 once school begins.

IMPACT testing will be for all incoming seventh graders and any eighth graders who did not play a sport for the school last year. IMPACT testing will be on Monday, September 8, 2014, at 2:30 p.m. in the media center. The testing takes about 1 hour.

A note on Random Drug Testing – High school students only

July 30th, 2012

If you plan to  participate in athletics, extra-curricular clubs/activities, or obtain a parking permit,  you are required to comply with North Warren’s Random Drug Testing program.  If you have previously filled out a consent form you DO NOT need to fill out another.  Only one form is necessary for your entire high school career.  If you are new to the school, or if you are an incoming freshman, you will need to fill out the Student Random Drug Testing Consent form and return the form to Mrs. Blanchette in the main office.
